Pre-office visit: Consent & Health History Forms will be sent to you to fill out prior to visit.
In office visit: Comprehensive consultation (30 to 60 minutes)to include initial treatment recommendations and personalized care plan; mini physical exam; review of medical history; labs ordered.
Follow-up appointment(s) - via telephone (20-30 minutes) $125.00
Review of labs; treatment recommendations/plan
Hormone pellet insertion $300.00
* Discount lab fees available for underinsured or uninsured patients
Established patient - in person annually (required) $199.00
Additional time or additional consult - via telephone $25.00 - 125.00
